Going Home: Patient Education Resources Project (PREP)

At Parker, we realize that many of our patients and family caregivers may need assistance with the skilled nursing facility discharge instruction. For patients and their families, we developed a video series, to re-enforce prior teaching at Parker, that emphasizes chronic disease self-management and medication management, for complex concerns.

We want our patients and family caregivers to feel prepared and confident, to manage their care at home. The video series provides patients and their families with easy-to-understand medical information, that you can access on ParkerVision (Channel 62), as well as Parker’s You Tube Channel, and on our website (www.parkerinstitute.org) from any computer, tablet or smart phone.

Each video takes about 15 minutes to watch and provides valuable information about managing a chronic condition and how to additional information. Please give us your feedback about our videos.
