About Us

Parker At Your Door’s Senior Care Management Program provides a full range of services to assist older adults and their families address aging-related issues. Our Certified Case Managers are Licensed Social Workers and Aging Life Care Professionals with years of long term care and case management experience. They are experts in elder care planning and care coordination.

Rates: The cost of services is $150 per hour for all work performed, prorated into 10 minute increments.
